The Planning Council and its subcommittees are governed by the following OPPs:

Members of the Planning Council and its Committees are required to complete the following forms at least once during their membership, and again if any information provided changes. Each form is able to be submitted either via the online form, or by downloading and filling out the PDFs.
Community members wishing to submit a complaint regarding deviations from an established, written priority setting or resource allocation process (e.g., failure to follow established conflict of interest procedures), OR deviations from an established, written process for any subsequent changes to priorities or allocations, can do so in writing by completing this form: Ryan White Part A Grievance Form
